I have a friend who is deathly afraid of needles..
He had a salvia test done and the dr said he had low T..the level that he would expect to see in an 70 yr old man.
My friend refuses to have blood taken so that is the only thing they know..
The dr gave him a scrip for Androgel..but he want sto see if he can raise his T naturally before uses the gel and shutting down his atestes...
Ideally he would go in for a full blood work up..but that ain't gonna happen..
so is there any way he can try to boost naturally...or being thatvery low( I do not have his exact number) is TRT the only way to go?

Sorry but saliva tests for T (testosterone) are not valid measurements of the vast majority of the body's T levels.
Ordinarily I copy-and-paste the science based reasons into this post, but since you're not undergoing T therapy, I'll leave it as a link.

You can only use natural means to boost T (Testosterone) to your genetic maximum tolerable level.
You can only use natural means to boost T (Testosterone) above your genetic maximum tolerable level for several days, at absolute most a few weeks. By then, the body's HPTA feedback mechanisms will have kicked in and used any one of several mechanisms at its disposal, to restore T to at-or-below it's genetic maximum tolerable levels.
Ideally he would go in for a full blood work up..but that ain't gonna happen..
Then go for urinary hormone measurements. Simple. Rheins Labs:

Actually..i am on TRT myself..i use Testim gel.
I will suggest to my friend the urine tests...that may help some and thanks for the other info..i will share that with him.

And I forgot to mention that the body's genetic maximum tolerable level for T (testosterone) diminishes every year after the age around 23.
In other words, your aging friend's genetic maximum tolerable level for T is now so low that using natural means to raise his T to that level will be a waste of effort.

But at his age the amount of GH boost he will need to will only be achievable with an injected substance.

I saw "70 year old man", but I now realize you meant a younger guy with the hormone levels of a 70 year old man.

I had ANOTHER doc show me labs yesterday with saliva way high, yet serums and 24 hour urines had him barely off the bottom.
He is on 30mg of T cream per day. "TRT lite".
Does this make sense to anyone? I see this all the time. How would anyone possibly recommend slaiva testing given these results? The docs who use them have no idea what they are doing, and have not given the concept a moment's thought.
The fact his doc thinks 30mg per day is sufficient tells us more. BTW, and this is what really counts, the patient still feels lousy.

I would suggest thorough blood testing first.
In case that his DHT comes high, I suggest that your friend,
before he take any other action,
buy box of 100 insuline syringes with needles (28Ga-1"long),
and poke himself daily (with empty syringes) until he finishes that whole box.
If he can't do that, there is no sense of trying to help him much more.
Possibly psychological sessions to get rid of needle-fobia first.

I think that doing 24hr urine testing at Rheins as a very first and only means of testing is a waste of time, money and (mostly) the minimal amount of good will that this individual have at this time.

Well, you can also "trick" you body, and avoid HTPA feedback mechanisms two ways, by
1. Cycling any products. I like the 5/2 method
2. Switching products up.
There are many things that boost T naturally. It is important to use a good brand.
I like Tribulus, Eurycoma, Vitex, Forskolin, Resveratrol, Nettle, Chrysin - All work to boost T naturally.
But all forementioned products boost T by stimulating a LH response, through several ways. Tribulus works on Dopamine, Eurycoma increases DHEA --> T, Chrysin inhibits T--> E, etc. they all works via different mechanisms, but have the same result - Increased LH.
Without normal, functioning testes (specifically Leydig cells) this is all a mute point.

At 29 he'd be fine on testosterone gel / cream provided his thyroid hormones were OK.
That's because poorly performing thyroid hormones will render transdermal hormone delivery a waste of time.
The kicker is that thyroid hormones can only really be measured from serum, not urine.
The urinary metabolite tests for thyroid hormones haven't yet identified all of the metabolites, so they don't yet correlate well enough to serum thyroid hormone measurements.
This means that his needle phobia will require his medical professional adviser to experiment with testosterone gel / cream without any indication whether this is even a good idea.

When we consider your friends hormone measurement options, if your friend is lucky enough to have adequately performing thyroid hormones, then your friend should do fine using just urine tests for the majority of hormones, and saliva tests for cortisol.

When we consider your friend's treatment options, if your friend is lucky enough to have adequately performing thyroid hormones, then your friend should be able to manage boosting his sex hormones and his stress hormones using transdermal creams / gels, and oral capsules - ie: sero injections - HOWEVER, his testicles will shrink to very small size within a few years, so he will be supplementing with additional hormones to make up the shortfall.
If he were prepared to inject HCG subcutaneously, into his belly fat (least stressful) then his testicles would not shrink.

None of us start out loving injection.
We all get over our injection phobia after a few months.
Initially I didn't want to look. Now I study the injection sites very carefully and I even pierce my skin very slowly to minimize the pain to close to zero.
NB: I now place the needle tip on my skin in a few places looking for a spot where there is essentially no sensation. Every morning I always find a new spot where there seems to be essentially zero sensation (no nerves within a few millimeters of the injection site).
I inject my HCG daily.

In my experience, using a 31g needle makes such an exercise mostly unnecessary.

Has he yet found help or decided to have serum or urine tests done?
I would say it is something you do not want to wait on to get help if you can afford it or have the medical Practioner or practioners to help you.
I feel that I waited a little long as I had lost excess muscle and lost weight then gained excess fat etc etc.
I though gave pleanty of time for things to go back to normal and just too much damage had been done from having low testosterone. It is amazig however dissapointing how the body re-configures itself.

Buy him a box of 31Ga insuline syringes (100ct)
Ask him to poke him self twice every day.
Possibly he can start with pricks that are used with glucose meters.
He have to overcome his fobia first.
