Good afterrnoon fellas,

On my last lab, my total T came back as: 1100, range 241-827.

I'm on the standard 100mg/wk of test, broken up into biweekly injections. I've been on TRT for 10 months and my T has always been well above range since starting. Is it unhealthy for it to be so high?? I imagine it could eventually cause some problems. Should I cut back to 75/mg a week?

Thanks

Great question! We have ample evidence of the health risks associated with low test, but very little on the flip side. Maybe Dr. John will comment.

You have not provided enough information.
Generally 100mg/week is either ok or even low.
TT=1100 is usually on a high side
But there is so many details.....
Devil is in the details.

First off, you (almost) are not supposed to look at TotalTestosterone.

Post your complete routine;
are you using HCG, when, how much
Are your bi-weekly injections equally spaced, post time
when was your blood drawn relative to the last T-shot
TT-1100
SHBG=?
E2(ultrasensitive)=?
DHT=?
Albumin=?
DHEAs=?

how do you feel?
what other known problems do you have?

Jan, unfortunately I don't all the values you specified:

Free test: 23.5, 8.7-25.1
E2: 20, <54
Albumin: 4.8, 3.5-5.5

My t shots are Monday and Thursday. HCG is 250iu Sunday and Wednesday. A-dex is .5mg E3D.

Blood was drawn 2 days after t shot. I feel good, but irritable at times, but that may be from lack of sleep, up at 4am. I think I'm starting to feel the effects of BPH-weak stream, frequent bathroom trips.

As asked previously

how do you feel?

good libido?
morning wood?
hard erections?
any ED?
lots of energy?
you feel relatively strong?

In general a lot of us, including me would like to have higher testosterone provided it made us feel better with better libido, good erections, high energy, etc.

Free test is still in high normal range.

How do you feel? Alright? If so, I wouldn't worry about it. 20 year olds, albeit not many but some, naturally produce up to 1100. I think that's the upper range?

You need some more test values in order to make a decision about your TT level. You need to determine your BAT. e2 should be ultrasensitive test.

Given your BPH symptoms. The following are important: DHT, e2 (ultrasensitive). There are a few good threads on BPH if you do a search.

An experienced urologist can feel your prostate in DRE and tell you relative size. They can do more sophisticated tests to determine bladder emptying (does it completely empty for instance). If you have a mild case they probably will prescribe an alpha blocker like Flomax to help with your symptoms.

That's a dead giveaway that your levels are too high and you need to cut back on something.

Estrogens can decrease and can kill all of these benefits it appears as well as body fat.

Post your other labs too- your body may be an unusual case which would explain why you may operate better at high levels.

We're talking bioavailable T (testosterone), or free T, as well as E2 (estradiol), SHBG, and DHT at a minimum before we can begin to justify maintaining such high levels.

But preferably we'd also like to see your cortisol levels (pref from saliva after wake, before sleep) and a discussion of your carbohydrate/sugars/oils intake (to make a rudimentary assessment of your insulin resistance).

Your hormones definitely do not operate in isolation to each other, and all these hormones listed above are very much dependent on each other, and contribute in a major way to determine why your equilibrium position is where it is.

if that range is not the youthful range then maybe you are right where you want to be. that range may be for a 39-59 year old man and personally i always stress wanting to hit the ceiling of the young range. theses ranges are not always offered on your test results and might be found searching that lab's tests on the internet.

if your e2 is 20 and free T at the top and the range is for a 20-29 year old then i would be thrilled, but that is just me. i feel the best where you are at. after experiencing an unexplained drop i was on top of with testing, 700-800 ng/dl felt like i forgot to take the doses.

you might want to consider your blood pressure too. if it tends to be normal thern maybe the urinary concerns need to be focused on, but unless tou are an older gentleman i'd be surprised the T caused that since you have only been using it for 10 months.

there are a lot of related tests to go with the ones you listed, but i wouldn't look for the perfect results on everything or else you won't enjoy being on TRT.

it's kind of like a baseball player adjusting his stance throughout the year to get comfortable at the plate. a lot of advice and shared experiences will be offered which is good but you have to face the curveballs of life with an approach you can feel good about so you're doing good to ask questions.

high T for me is like a power hitter who hits a lot of homers but has to cope with the strikeouts (irritability) a little more than others. think howard of the phillies.

over a year ago i felt like a player whose career was coming to a pre-mature end who should have had a lot more hits in him but had lost his passion for the game and wouldn't reach his career goals.

now i feel like i just got signed to a lucrative multi year contract and have a lot of productive years left, only i will not get banned for using T and hgh.

hope i am making sense and not just being silly. only you know being which kind of batter will help you achieve your goals the rest of your life.
